Unlock Hard Mode

Beat the game, then hold L when starting a new game.
Unlock Jackson

Beat the game in Hard Mode with an A or S rank, then use
the saved game when starting a new game to unlock Jackson.
Unlock Ultimate Mode

Start a game with Jackson. Beat the game using all elves then,
when starting a new game, hold R to unlock Ultimate Mode.
